Combined orthodontic-orthognathic treatment: what can our patients teach us?
Combined orthodontic and orthognathic surgical treatment can provide very favourable results with regard to aesthetics, dental occlusion and oral function. When deciding on orthognathic treatment need, orthodontists and maxillofacial surgeons tend to give substantial importance to the soft tissue profile. Are our patients however actually aware of their profile, and if so, at what age do they gain this awareness? Moreover, in patients who comprehend the benefits of orthognathic surgery and chose to undergo this treatment, what are their experiences? It is crucial for us to understand the perceptions and experiences of individuals having undergone combined orthodontic and orthognathic surgical treatments. A better understanding of patients’ views can increase our awareness and help is in multiple ways. Firstly, this may help in individualised decision making with regard to orthognathic treatment, which should be patient-centred and consider the patients’ perspectives. Equally, it will allow us to properly educate and counsel our patients and their families throughout the entire treatment process.

Dr. Chaushu is a member of the Editorial Board of American Journal of Orthodontics and Dentofacial Orthopedics and of Orthodontics, Associate Editor of Seminars in Orthodontics and past Associate Editor of Progress in Orthodontics.

Orthodontic treatment of impacted teeth cannot be a “cookbook”. Although the results can be excellent, treatment is complicated, usually long and requires one or several surgical interventions. Manifold reasons can cause failure, such as inaccurate positional diagnosis, missed resorption of adjacent teeth, wrong mechanotherapy, surgical mistakes and more. The risk of failure increases with the severity of impaction. In this presentation, several difficult impaction cases will be utilized to illustrate the step-by-step decision-making process which can help the clinician make the most thoughtful treatment decision, highlighting the importance of early diagnosis and timing, gathering of comprehensive pretreatment data, assessing treatment alternatives, weighing the evidence and critically reviewing the decision and its consequences.

Orthodontics in 2023: Can we promise our patients improved precision and a better outcome?

Technological advances are rapidly changing the way that we practice orthodontics. Digital scanners, 3D-printed plastic aligners, artificial intelligence, digital workflow, 3D-printed customized fixed appliances have become part of our everyday lingo, and have paved the way for a different delivery of orthodontic services, and a different patient experience. To what extent does this represent an improvement over what we used to do 20 years ago? Have these advances resulted in higher quality outcomes? What will orthodontics look like by the end of the 21st century? Will orthodontists eventually become redundant and be replaced by AI? Dr. Daskalogiannakis will attempt to address some of these questions, in a thought-provoking presentation.

He is an active member of multiple orthodontic societies, and lectures widely. He and his very accomplished wife have 3 amazing daughters.

Orthodontics has been using the rectangular-slotted edgewise appliance to treat malocclusions for 100 years. The core problems built into this appliance center around the “play” or “slop” between the bracket slot and any arch wire it engages. This means that inaccuracy is inherent to edgewise. As clinicians, we need to manipulate our bracket placement or arch wire shapes as well as utilize a range of arch wire sizes/shapes and guess how much over-correction will be required to proceed in treatment. A triangular slotted bracket system which fully engages arch wires without “play” while producing significantly lower frictional forces resolves all these issues. The TRIAMOND system does this using only 2 arch wires and permits resolving any malocclusion from the day of bonding. It does so without the side-effects inherent to all other appliances. Tooth movement becomes more predictable as well as more accurate. This is accomplished using less force and a reduced overall treatment duration because time is not wasted on correcting or avoiding the unnecessary problems, which are a part of edgewise, while achieving precise resolution of any malocclusion.

Lessons not learned from the introduction of new materials in Orthodontics: the case of aligners and 3D printing

The lecture discusses the introduction of new materials in the profession in the past 30 years and examines the unsubstantiated claims accompanying their marketing: the list includes superelastic NiTi wires, moisture insensitive adhesives, self-ligating brackets, self-etching adhesives, and focuses on the use of aligners to treat any type of malocclusion, and the hype of 3D-printed appliances. Evidence from the literature reports the actual efficiency, efficacy and safety of aligners and 3D-printed materials as well as the mechanisms underlying their mechanical and chemical alterations in vivo and resultant clinical implications and biological properties. The growing complexity in assessing the validity of evidence is also discussed, in the context of the increasing number of pseudo-scientific periodicals, industry-sponsored symposia, predatory open access journals where anybody can publish anything anytime, and industry-paid speakers, whose remarkable aversion to publishing has not prevented them from lecturing around the world.

Orthodontic clear aligners can now be fabricated using 3D printing technology, thanks to the availability of 3D printable photocuring resins. Compared to conventional thermoforming aligners, 3D printed aligners offer several advantages, including greater precision and customizability in design and manufacturing. By skipping the intermediate steps of fabricating resin staged models and thermoforming clear plastic sheets, potential cumulative errors can be minimized. Moreover, 3D printed aligners can change the fundamentals of clear aligner biomechanics by allowing greater control over the geometry. They can also be more environmentally sustainable by reducing waste generated from printing resin models and thermoforming plastic sheets. This presentation will introduce the digital workflow for manufacturing 3D printed aligners, from creating aligner shell files to post-processing procedures, and explain how to:

  • Understand the differences between thermoforming aligners and 3D printed aligners
  • Recognize the biomechanical advantages of 3D printed aligners
  • Develop a treatment strategy that utilizes the features of 3D printed aligners.

Orthognathic Surgery to correct severe malocclusions and skeletal deformities involves a considerable amount of treatment planning and coordination with a multidisciplinary team. The success of the surgery requires an excellent collaboration between the orthodontist and the surgeon primarily, and secondarily with other specialties that may be involved during the diagnostic, treatment, and posttreatment phases. There is a recent movement into the “surgery-first” approach, which eliminates esthetically undesirable facial changes due to decompensation of the teeth from the presurgical orthodontic preparation. For both the conventional and “surgery-first” approaches, careful and detailed creation of a treatment plan is crucial to produce the most accurate, esthetic, and functional results.

TITLE: Surgical first and Invisalign System: Combined Digital Approach

SPEAKER: Simonetta Meuli, DDS, Ortho Spec, PhD

ABSTRACT: The aim of the present work is to verify the correct results in dental and skeletal stability in terms of symmetry, occlusion and functional balance by the association between the Invisalign system (IS) and the Surgical First (SF) technique. Cases of severe class III malocclusion (treated with association between IS and SF technique) followed an orthodontic-surgical workflow: intraoral scanning; OrthoCad with JA-control and Occlusogram; orthodontic planning with ClinCheck; Dolphin software to plan skeletal movements and correlate these to aesthetic results. At the end the patients present an Angle class I occlusal relationship, no deep bite, no scissor or cross-bite and no open bite. No TMJ discomfort nor severe post-surgical complications and no surgical orthodontic relapse occurred. The surgical and occlusal results are stable in all patients. The results suggest the validity of this association in management of class III malocclusion patients. The combination of SF and IS provides the clinician with a minimally invasive alternative to manage severe dentoskeletal disharmony without having a negative impact on the quality of life perceived by the patient.

Growing patients with the Invisalign System

Orthodontic aligners are the treatment of choice in growing patients more and more frequently, thus it is essential to have a good knowledge of the complex mechanisms of craniofacial growth and the protocols that Align offers for each type of malocclusion. In her Lecture, Dr Meuli will talk about interceptive treatment in the mixed dentition with Invisalign First, its indications, limits and possibilities. She will discuss how to approach the patient in mixed dentition and present cases with crowding, spacing and eruption problems. She will also talk about occlusal and functional interferences, how to identify and correct them.

Moreover, she will go deep into the Clinical Preferences and Invisalign First Protocols:

– expansion with Invisalign First and control of transverse dimension
– anteroposterior correction protocols: from Mandibular advancement to the use of elastics and distalization
– control of the vertical dimension

In a Class of their own!

Although Class III malocclusions are the least frequent, they are frequently harder to treat and more likely to need orthognathic surgery for the best treatment. The reported incidence of this malocclusion ranges from 1% to 18%, with Asian groups experiencing the greatest prevalence and Caucasian populations experiencing the lowest. Generally speaking, class III malocclusions can be divided into two groups: developing and non-developing. Early intervention utilizing two phases of therapy is frequently recommended for Class III malocclusions that are still developing, with children (ages 7 to 9) showing more orthopedic benefit. Several approaches have been suggested for treating adult Class III patients who are not developing, including multi-brackets with Class III elastics, extraction treatment, full arch distalization, and multi-loop edgewise therapy. These procedures aid in secure occlusions and appropriate interincisal relationships, but often require either intraoral or extraoral anchorage to retract the mandibular incisors. The patient’s compliance is frequently crucial to the effectiveness of the treatment. This lecture will focus on identifying and comprehending the aspects that play a critical part in strategizing non-surgical treatment options for Class III malocclusions, as well as demonstrating the concepts utilizing an eight-point protocol to increase the efficacy, aesthetics, and stability.

Τίτλος: Ποιες είναι οι απαραίτητες αλλαγές που θα καταστήσουν τους χώρους εργασίας στο Ορθοδοντικό Ιατρείο πιο εργονομικούς;

Presentation Title
“Orthodontist assistant in 2023: classic values and innovations”

The clinical orthodontic practice is now part of 4 handed dentistry. In the modern orthodontic practice, the contribution of the dental assistant to the improvement of the quality of services is indisputable. Today, the orthodontic assistant must be sufficiently qualified and familiar with all modern orthodontic techniques. The theoretical training in the specialty of Orthodontics aims for the assistants to be able to successfully fulfill the clinical and secretarial requirements of an orthodontic office. Specialized orthodontic assistants should recognize the basic orthodontic malocclusions, be familiar with orthodontic instruments and appliances (fixed and mobile), have an in-depth understanding of the steps involved in clinical orthodontic procedures, and finally be in position to communicate with patients and parents and explain the procedures and goals of orthodontic treatment.

EU Regulations & Directives: application and practices in the orthodontic office

The aim of the presentation is to encode the basic information regarding the application of the General Data Protection Regulation (2016/679) and other regulations and/or Directives that will apply to dental practices. We will report on the provisions & requirements
pertaining to the storage and processing of data subjects as well as the provisions of the Greek Dental Code of Ethics (GDCE) that are pertinent to the EU regulation.

The dental offices are obliged to handle personal data under a lawful basis and patients should be informed that their personal & sensitive data are:

1. stored using the highest-possible privacy settings in an inaccessible place so that the
data is not available publicly,
2. processed under a lawful basis,
3. used solely by the orthodontist or a designated person fully anonymized,

Also, their medical & dental history, and their diagnostic records should be kept for ten years after competition of treatment according to articles 12-14 of the GDCE.

The «European Health Data Space» regulation is expected to be implemented in January 2025. It is required that primary data of the HER file including diagnostic records, are interoperable to be transferred in a central EU cloud type server.

Additionally, if the orthodontist is characterized as final manufacturer the consequences from the full implementation of Regulation 2017/715/EU (expected in 2027) regarding medical devices will be highlighted. Finally, the framework of the “European Green Deal” is
enforcing a circular economy where the single use non-recycled items are gradually banned.
